Before your child can begin our program, the required forms need to be returned at least 2 days prior to the child’s first day of attendance.
Required Forms
- Parent/Child Information Form
- Physical Form (KDHE-Medical Record): Must be completed by a licensed physician or qualified health person.
- Shot Record (Immunizations): A plan must be made if immunizations have not been started or completed.
- Emergency Medical Form (KDHE)
- School Lunch Form (CACFP)
Change in Information
If there is a change in your work schedule, address, telephone number, place of work, hours of child care needed, etc., please notify the office IMMEDIATELY. You must be available in case of an emergency at all times.
Picking Up Children from the Center
You are required to list people who have your permission to pick your child/children up from the center on your enrollment papers. Specific forms must be completed for this purpose. If at a later date you wish to add someone to your pick-up list, please notify the office in writing so that we may update your child/children’s file.
Anyone on the list will be allowed to take a child from the center at any time. If someone not on the list comes to pick up your child, we will contact you ASAP. They will not be allowed to take the child until we hear from you. If you are divorced, we ask that you provide us with file-stamped court papers stating the custody agreement if there are any questions or concerns.
